All insurance coverage will be verified prior to your surgery. You are responsible for any Insurance 'deductibles', 'co-pays', and 'co-insurance' amounts. If you have any questions, please call at 214-647-5366 to speak with an insurance verifier.

Prior to the Day of Surgery
- If you need to make financial arrangements, please notify us prior to the day of surgery.
- Patients who are paying cash for their procedure, must pay the entire fee prior to surgery.
- Acceptable forms of payment include cash, personal check, American Express, Visa, MasterCard and Discover.
Day of Surgery
- Bring insurance card (s), co-pay/ deposit (if you have not already paid)
- Must bring photo ID (drivers license or ID).
After Surgery
- TIS will file a claim with your insurance company after the completion of your procedure. Cosmetic, self-pay and CareCredit are due at time of service.
- TIS will bill for the facility's fees only. You will receive a bill from others who have participated in your care, including your surgeon, anesthesia provider, pathologist and radiologist.
- Acceptable forms of payment include cash, personal check, American Express, Visa, MasterCard and Discover.
